There's no overlap from the first volume of the OST, I think the few radio songs on the last disc might be repeats, but they're repeats from the radio collection CD, not the first volume of the OST. It's all the DLC music, as well as other stuff like the Assassin's Festival and the music they added with the free extra story content (ex. Ravus' Last Moments) that they put in after the first OST release.

(Though I think you got Moogle Chocobo Carnival! and Fireworks Finale's composers mixed up - it would make more sense for Uematsu and Shimomura to have remixed their own works rather than each others, and indeed, google translating the names on those tracks on the mora.jp page give me Uematsu for Moogle Chocobo Carnival! and Shimomura for Fireworks Finale)

I guess the piano tracks are footage from the actual concert. Someone has to extract the audio from it.
